Transition Care and Assistance

A strong support system at home, along with coordinated care, can reduce one’s risk of hospital readmission. At Home Sweet Home Care, we have processes in place to help people successfully transition home after a stay in a hospital, rehabilitation center or skilled nursing facility.

Our transitional care services can help with the following:

  • Review hospital discharge instructions and incorporate these instructions into a client’s personalized care plan

  • Pick up prescriptions, provide medication reminders and help the client adhere to their prescribed regimen

  • Educate the client, caregivers and family members 

  • Assist clients with scheduling and attending follow-up medical appointments

  • Communicate with the client’s family members, discharge planner, health coach or other health care professionals

  • Assist with personal care services such as bathing, grooming, personal hygiene, toileting and mobility 

  • Help with companion care services including preparing meals, laundry, light housekeeping and errands

  • Identify trip hazards and recommend home safety solutions to prevent falls â€‹

  • Call a friend or family member to let them know their loved one has arrived home

Hospice  Care assistance

Home Sweet Home Care caregivers work alongside the hospice team to ensure individuals remain comfortable, safe and peaceful while remaining in the place they call home.  Our hospice care assistance includes:

  • Assisting with personal care including bathing and dressing, ensuring oral and personal hygiene needs are met 

  • Providing companionship to the client and the family members involved in their care 

  • Offering respite care so that family caregivers can take breaks 

  • Providing medication reminders, as well as creating an environment of comfort and peace as the patient manages their symptoms and receives hospice care 

  • Helping with errands and other household tasks, such as light housekeeping, grocery shopping and meal preparation, so that family caregivers can focus on the care and comfort of their loved one
